Visa extension for my spouse

Hi everyone

I am applying for my wife's visa extension under the 5 year rule. Few clarifications are required if anyone can provide their expert opinion.

Firstly do I have to pay the IHS.

Secondly does she need to give an English language test again as she gave IELTS with the first application.

Finally is there a time limit on how early can one apply for a visa extension as my wife's existing visa is expiring in July.

FYI the initial spouse application was made in Dec 2012 from Pakistan and my wife came to UK in mid 2013.

Yes, it is £500
Secondly does she need to give an English language test again as she gave IELTS with the first application.
She needs to meet ALL the same requirements as when she initially applied.
Finally is there a time limit on how early can one apply for a visa extension as my wife's existing visa is expiring in July.
If you apply too early, she will have a shortfall for ILR and will need another extension. You don't say when her visa expires or what the start date is.
