In a previous thread I was pointed out to this resource:

to request an employment history from the HMRC. Unfortunately this page doesn't have a link to the actual form. I suspect it is this one:

However I am bit confused because:
1) this page mentions "national insurance". I already have a statement about my national insurance for the last 17 years and it doesn't have a breakdowns of all employers.
2) My employment history is too long and won't fit on the form. Can I point to a separate attachment? According the first links I posted:
I fear that they will reject my request if it doesn't fit the form.. Please note, HMRC will only accept requests made on this form. ‘Incomplete forms will result in the correspondence being returned to you’.
3) On the same page it is stated:
Is this true?. For residency purposes HMRC provide National Insurance and benefit information for the last fourteen tax years.
