Spouse Visa Extension (FLRM)

Hello every one

I was wondering if some one can put me on the right path. Any advice will be highly appreciated by me and my partner.

Scenario:
I am a British Citizen and married to a Pakistani wife who is living with me in UK on Spouse visa. She was granted spouse visa in September 2013 until May 2016. Now, its time for visa extension due to 5 year rule. When she entered UK, there was no health surcharge and no biometric registration requirments either. Now, that I have downloaded FLRM form all these requirements are there. So, I have the following 3 questions;

Questions
1- Does she need to pay heatlh surcharge? If so, when and where? please..
2- Do we need to make arrangement for Biometric registration before making the application or will it be done along with the application process. If before, when and where? please..
3- Finally, does my wife need to do her IELTS or equivalent again? She did IELTS academic before coming over to uk which is only valid for 2 years.

An early response can save us a lot of time. Thank you very much :)

1. Yes, it is £500 and you pay online. See Immigration Health Surcharge (click)

2. If you apply by post, HO will send you a letter for biometric enrollment at the Post Office (there is a fee for this, £19-20).

3. If on the list of approved test providers, then it should be ok.

thank you for your reply cr001. I am not sure about your answer tto questions. my wife did IELTS from British Council in Pakistan which is valid for only 2 years. it's her 3rd year since she obtained spouse visa. Does she need to do IELTS or equivalent again? please advise. thanks.
