Need some help / info on EC

Although I am sure that there are many questions already answered on EC, I could not find the one i am looking for.

Do I need to submit a reference letter / a statement stating my roles, responsibilities etc from my current employer?

Also, I have some questions specific queries...like what do i write in VAF1 form question
5.1 duration of stay..1 or 4 years?
5.4 whether u have any relatives...if i write yes, does it adversely impact the case?,
7.1 intended job...can i attach an annexure?,
7.2 Name of employer - ?? what to write
7.3 - going as a workpermit holder - yes?

If these are already answered elsewhere...can someone point out that link?
