Name mis match .. urgent help needed

Hi friends ..

I had a issue with my name.
In all my educational documents exept PG my name is written like " DEEPIKA REDDY S ".
And in the next column my father name was written as " RAM REDDY S ".

Unfortunately in my PG certificate there is no column given for father name .. so in this certificate
my name is written like " DEEPIKA REDDY S RAM REDDY S " (My name is written including with my father name).

In passport my name is " DEEPIKA REDDY SAMA " (SAMA is my surname).

I applied with this documents and got approval. Now im applying for EC.
I have read some where in the forum in a situation like this we need to submit a Affidavit for name change.
Is that true (or) is it fine if i apply for EC without affidavit.
