EEA family permit for extended family member

Hi.
This forum is really helpful. I need to ask that i am non EEA national and i have EEA2 card valid upto 2018. I want to bring my brother in UK. I have been supporting him and I have the proof of supporting them. My question is

Can he apply as EEA extended family member visa?
If he can apply then in visa application form where we enter EEA family member details, should i enter my details or my wife(EEA national) details

an idea what is the outcome
