EEA PR? form. Work history and travel what number of years

HI,
I am originally from Poland and have lived in London for last 14 years without any major breaks (holidays etc only). I had never though of applying for citizenship as I did not need it. Of course, recent events made us panic a bit and we would like to do it.
I have learnt I need to apply for permanent residence first. I have a few short questions and would appreciate some info:

1. Want to be sure the right form is used - EEA (PR)?

2. IF qualifying period is 5 years and I have lived here 14 - do I need to state my employment history since my arrival date in 2002?

3. Likewise, do I need to state every single trip out of UK within last 5 or 14 years? It is going to be extremely difficult for me to find exact dates of travels that far in the past.

4. I am a bit confused about the employment history too. I arrived in Nov 2002 on a student visa. In 2003 I worked part-time and the company then offered me full time which I accepted. Long story short, I thought It was down to an employer to sort out any legal requirements then so I worked for them Full time for about 8 months while studying. I then studied only (no work) for a few months before EU accession date. In 2004 June I was hired and applied for WRS. Completed 1 year and got a residence card. Since 2005 I worked continuously until 2013 when I was made redundant and started my own business soon after.
My question here is really - is there a way of HO checking whether I worked full time or part time at the start of my life in the UK? Anyone risked omitting some details from their work history or is it being investigated in details?

5. I have a child - born here in 2012 and she has British passport based on my 5 years of life in the UK before she was born. I remember at that time I had to provide just recent 5 years info so I did not have to go back to the start of my life in the UK.
Firstly, there is no option to state on the form that I have already provided my 5 years of life and daughter has her passport rights proven this way. Is there a way to include this information anywhere? Does it matter at all?

Thank you. Hopefully that was clear enough. I want to fill the form right as apparently they get rejected easily. Any further advice much appreciated.

1) Yes. Although its not a mandatory, legal requirement to use that current 'monster' form.
You could use an earlier (simpler) version of form.

2) it depends where, within your 14 years time served in UK, your qualifying period sits; at beginning, in middle or near the end.

3) Yes.
Because HO want to determine if you have maintained continuity of residence in UK;
and, if you have acquired PR, whether you have lost it (due to absence).

Try your best. Month/year will do at a pinch.

4) Sounds like you may have acquired PR at some time in 2009.

Its not just HO crosschecking your records with other government (and other) agencies.
Read the Declaration you have to sign vouching for the veracity and completeness of your information.

5) You can add a cogent cover note if you wish.
No, it does not matter - your application will be assessed on the evidence submitted and not waved through on the basis of your child.

Could you just clarify point 2 for me. I have lived here 14 years non stop. Which 5 years would be my qualifying period? I assume last 5 years. So do I provide employment history and evidence for the last 5 years?

The period is not necessarily the last 5 years.

Your qualifying period is one where you have exercised treaty rights continuously for 5 years - and have rock-solid supporting evidence & have complied with WRS (as necessary) & had CSI (if ever a student in UK).

Review your timeline;
Check dates of any prolonged absences from UK;
Read the guidelines;
Look at your documentary evidence;
- then you can see when you have acquired PR.

Note
The DCPR you are now applying for is just a confirmatory document - if you have PR (settled status) you have it already!

Do I understand this right that I do not have provide last 14 years of employment status (that is a basis of my legal stay here) but selected 5 years? So I am self employed right now and it is more difficult to prove my years - I need to collect documents I do not currently possess - need copy of tax returns, NI records etc, but before that I had been employed continuously 2004-2013. WRS sorted 2004-2005 and residence card received (NOT PR, just residence card). Can I then provide evidence for years 2007-2012/13 instead of last 5 years?
