Applying for HSMP Pls verify the list of evidences

Hi all,
I am planning to apply for HSMP and planning to submit the below documents . Pls clarify some of the doubts I have.

Prev Earnings section
1. 12 Month Salary slips, signed and sealed. claiming for Jan-2007 -
Dec -2007
2. 12 month bank statements(ICICI). The salary credit transaction shows
INF/0 SALARY FOR NOVEMBER 2007
3. Original ITR filed acknowledgment form.It does not cover the entire claim period.

Education.
Original Degree certificate (B.E computer science from University of Madras)
English Proficiency
Medium of Instruction letter.

Age Proof.
Original Indian Driving license (CARD type ).

Are these docs enough. if so pls clarify some of my doubts regarding this.

1. regarding Bank statement-- My salary transaction does not mention the name of my employer, Is that a must.
My salary account no is specified in my Salary slip, is that fine.
2. There is no date in my salary slip only Month .if so will they take the conversion rate on the date in the bank statement.
2. Is my Driving license is enough or should i submit a notarized passport copy.

Hi UKHSMP,

In your earnings document , you have to provide the employer certificate stating that your earnings from Jan-2007 to Dec-2007.

is the salary certificate a must, becoz I might not be able to get that.

50. If your income tax return does not cover the entire period claimed, then you should provide either or both of the following:
• Your bank statements covering the period claimed for;
• A letter from your employer on company headed paper, stating your salary. The letter should confirm that you have received the salary stated for the earnings period claimed.


In your case, I hope bank statements are good enough if you cannot get that.
If you have birth certificate for Age proof, please submit that.

Employer letter

Hi,

Iam also showing my income from Jan07 to Dec07 and my employer is not ready to give the letter for all the months. I requested them and finally they told they can give only the Annual net and gross total figures with my name and employee number in that letter.

Will this letter work or there any other options. Thanks.
