can we extend sposuse visa earlier than 28days?

my wife granted first entry clearance 5/4/14 - 5/1/17
she entered UK 4/5/14, so to reach the 2.5 year will be 4/11/16.

can she apply extension at PEO on 11/11/16? as we are going abroad for about 1 month so wont able to apply in the 28days period as suggested.

Also do you have to pay ALL the fee online when booking PEO apppointment? ie surchange, visa fee, £500 fee?
