EEA PR Card Passport photo question please?

HI guys I'm slightly confused. it sais in the form to attach 2 passport size photos and last time I did this form I remember I had to put name on it but I don't remember if I need to put full name full address and sign it or just name and sign it or just full name can some one please help please?
