Form AN/ Employer address

Hello all,

I've searched forum but couldn't find the answer..

1. Question 1.50, the column about employer address. Not sure what to put there, address as per P60 (head office registered location in London) or physical address of an office in another city where I've worked?

2. I've worked for a company ABC, but then company changed it's name to ZWG, My payroll number stayed the same, head office address stayed same as well. Do I write down as working for the same company, or it will be classed as 2 different employers?

1. Suggest you put the address you use for HR contact (probably Head Office), which may not be the same as your work address, as you say.
2. Use latest name of company - maybe add note that company changed name.
