I was trying to find link for "HSMP request for review" application form on http://bia.homeoffice.gov.uk/workingintheuk but for some reasons can't find.

I could find some unofficial download links but not any official one.

Can someone please help?

Hi Friend,
The form is no longer available on their website. But u can apply for review using this e -mail HSMP.workpermits@ind.home.office.gsi.gov.uk or u send mail to the address where u sent ur initial application.Note that u will not be allowed to send any document apart from the application sheet. Remember to argue your case making reference to the officially published guidance for applicants.
