received back passport only - no decision letter or

Hi everybody,
i am new on this forum, Recently i applied for eea family visa(my father is italian and he is currently living in uk) for uk,i put all docyments for example birth certificate, bank statment and work contract of my father.
i made application on visa4uk and made appointment in Rome Italy,
after 20 days they returned me my passport witout any visa stamp and without any refusal letter, i got only passport.
is this happened before to anyone?
