Question 7.4 on MN1 Form

Hi All

Can anyone tell me please if I need to complete 7.4 on MN1 regarding Guardian since I'm the father and the guardian of the child and have already signed the consent in 7.1 as father and 6.19 as guardian , I'm not sure what I should do or shall I leave it empty ?
