No, just leave it2. In new application (started from september, 2017 i presume), It is asking to enter "Do you have a valid national identity card?"

Send all passports, just safer side4. While filling the application for me, wife and children, it only asked information of the Current passport. I have filled the form with the current passport details and submitting this Passport with the application.
at one point I need to pay NHS surcharge during application but they are updating online application questionaries all the time. I have applied in August end.In the online application, it does not ask for NHS surcharge details ?

optional, I did not send an envelope but they returned in a well-packed plastic envelope.Also include another recorded delivery envelope (along with the documents you are about to submit) so UKVI can return all your passport(s) safely.
