Please help regarding HSMP extension under Tier 1

I initially got HSMP for 1 year and then extension for another 3 years. To complete 5 years I need another 1 year extension and I am really confused now with all the changes. Can anyone please tell me step by step what to do next. Do I just fill in tier 1 application form or do I need to get hsmp approval again. Please help me as my extension is due next month

I am on the same boat. Home office don't like us more and playing with us. Now must have to qualify 75 points in tier1 for extension.
