SET(M) is correct form someone settling from FLR.1) What form should I use for ILR, after reading all documentation it looks to be SET(M), but would be good if someone can confirm.

What level CEFR is her old test? A1...? Likely the test has already expired too. She needs B1 for ILR.The IELTS was general category
(Gave it for entry visa) and was given from an approved provider at that time. I am not sure if it's is still an approved center. It's been used to get visa first time and for an extension.

Settlement checking service is for POSTAL applications and is completely separate to premium service appointments. You do one or the other, not both.Also i am thinking of going for premium same day service and settlement checking service. Do these need to happen on same day? Or i can get settlement checking service done few days before by appointment for premium same day service. I know for postal service, documents after verification are posted straight to home office.
No, that is not an accurate assumption for dependents. Main PBS visa holders yes. Because the levels required for spouse visa and ILR and citizenship are different.It doesn't makes any sense, if you have qualified once, why you need to give it again.

An applicant may decide to apply in person at a UK Visas and Immigration Public Enquiry Office (PEO) and would pay for a premium service. The settlement checking service is available as part of the premium application route. Merton Settlement Checking Service will check the application form is complete, correct supporting documents have been provided.
I will post details here, I think that is a good option.
