Indefinite Leave to Remain in the UK (5 year route)

1. Which form should be filled out for application for an Indefinite Leave to Remain in the UK (5 year route) as a partner of a British Citizen?
2. For the indefinite leave to remain application, can it be submitted as soon as it is 5 years since the applicant has lived in the UK or does the applicant need to wait until 1 month before the current residence permit expires?

You can apply no sooner than within 28 days before the 5th anniversary of the date you entered the UK in 2013.
