Confusion Over Indefinite Leave to Remain Self Assessment document

Hello Everyone
I am applying for ILR this week. I need you urgent help and advice on this issue/confusion i am facing.
I am taking the 5 years route as spouse of a British citizen. On Previous visa application and extension, I have held a permanent job with a company and therefore had payslips and bank statements until recently.
However 3 months ago (ie from November 2017)i started earning as a Limited company, me being the sole director.
I have all the payslips and bank statements for the months prior to that. However, i am confused as to how to put for my salary and income for last three months where i am self employed. If I consider the amount earned for the whole year (ie 2016 dec-2017 dec), i would be able to meet the £18500 requirement.

I have contacted my accountant to give me a letter of my earnings for last three months.

1. My question is, do i need Self assessment form for the last three months( ie from November 2017 onwards) ??
2. Is the accountant letter stating my salary and source of income enough instead of a payslip for the time since November 2017.
3. Do i need to give my company registration documents along with the application. Any other documents from the company side?

As i could not find any slot for premium service, therefore i will have to post the application. So i want to make sure i send all the relevant documents.

I WOULD REALLY REALLY APPRECIATE ANY ADVICE OR HELP. Thank you

It would help if you could confirm under which category of leave you are applying for ILR :?:

my wife is employed too. However, i just want to use my earnings alone without going into my wife earnings.
