Overstaying always matters. It will exclude her from applying for Citizenship for 10 years from the date she now gets a new visa, if she gets one.
FLR(FP) will also put her on a new 5 year residence period (if she meets the English and financial requirement) or a 10 year route (if she doesn't meet the requirements). There is no guarantee that she will be granted a visa.
She has no access to free NHS and should be paying for her NHS usage at 150% of cost.
Currently these applications are taking many months, sometimes longer than a year to decide. Often refused.
She will have to pay the visa fees and the Immigration health surcharge of £500.
