FLR m initial 2.5 yrs spouse visa

Hello im applying for FLR (m) initial 2.5 yrs, as im already married on 11th june 2018 im in a uk for fiancee visa,my concern is i am applying in FLR m manual form which is hand written can i ask if how do i pay for visa?it stated there a payment detail if i pay through credit/debit card i should fill up if im gonna pay was it sent all together with the visa form?where i will sent if ever i will get premium service?and how about the IHS and biometric?do i have to do it first before filling up and sending the form?i hope someone can help
and can i still use my english test which i use for fiancee visa it Ielts life skills A1 last nov.10,2017,the body awaded is cambridge english your response will be appreciated!
and can you help me with this question 6.8 Add all addresses you and your sponsor have lived at in the 2 years preceding this application if you have lived at your current address with your sponsor for less than 2 years?was it my address when i was in philippines?im in a fiancee visa right now and applying for extension of initial 2.5 yrs.thanks alot.

You pay online if you are booking a premium service in person appointment. Or if you applying by post, you complete the payment form in the FLR(M) form.

You pay IHS online, whether you apply by post or in person. https://www.gov.uk/healthcare-immigration-application

You need English A2 for your spouse visa.
