advice on EEA National for letter confirming presence in the UK

Hi, i am filling in my application for British Citizenship after having my permanent residence card -
I changed employment several times so not sure a letter from my latest employer could be used -

as they are talking about NHS - should i ask my GP practice a letter certifying i have been register with them for the last 5 years? (even if that doesnt cover the beginning of my stay in the UK? )
