Residence card application

Hi guys, i started filling the form for residence card today. In short i came to UK as an extended family member of a EEA citizen fith family permit. I am in a relationship with my girlfriend for almost 6 years now. I am filling everythin well but i got stuck at section 5. In section 4 at the beggining it said if i am an extended family member and umarried partner i should go directly to 4.4 and than to section 5, but at a point 4.4 i ticked second option and it said to finish section 9 and than 10-15, than 16,17 and 18. So, should i do the section 5 or not? It looks rather important as it is a prt where we confirm our relationship.
I would really apreciate any help about this...

Also i forgot to add i moved to UK beggining of this month but we lived for more than 2 years in appartment in Belgrade, Serbia so i guess in section 5 i have to enter that address to confirm we were living together for more than 2 years?
