HSMP (EC) - Which application form to use?

HSMP (EC) - Which application form to use?

Dear All,

I got an approval for HSMP on 28Jan 2008 (this was under the old scheme). I couldnt apply for LTR in UK and had to return to my country of origin. I now wish to apply for Entry Clearance. I was wondering if someone could advise on the appropriate form I have to use. Should I use the old Entry Clearence form or is there an alternative form in the new scheme I can use.

Also if someone could advise on the supporting documents I have to use. I have emailed the HSMP team in UK but havent yet heard from them.
