IHS Surcharge for tier 1 Dependant - Main Applicant applying for ILR

Hi All ,
I am applying for ILR under tier 1 and My wife applying for PBS dependant as she has not completed her 5 Years in UK.
I have filled the online form and it just asking for One document to support the application-( Bank statement for the proof of Maintenance) . Is this the only document I need to upload or am I missing anything else ?

She was living in india and and after the our marriage her surname changed and applied for new Indian passport with the amendments . In the application there is one question , have you ever changed your name ? do i need to declare the change of her surname ?

I am filling the pbs dependant application form and at End of the application it taking me to new page to give the details to pay the IHS, i gave all the details and total to pay showing £ 0 . My understanding is she will get a visa for 3 years and i have to pay IHS £1200. is anyone having same issue to pay Ihs or any helpfull tips to sort the issue that would be great help.

That would be because you are entering your current visa expiry date.

Hi ALL,
As I have already mentioned above the issue I am having for the IHS payment for my dependant . Now the PBS Dependant application form itself has the link to pay the IHS surcharge , its giving the total to pay 0 ,I doubt same as CR001 mentioned it may be because of my visa expiry date is in this month .

I created an account in the IHS website and after filling the PBS dependant details its have three options to choose. ( Please see the attached picture )
1. Are you applying to join or remain with a person already in the UK ?
2.Do you have any dependants ?
3.These Details are correct .

Actually i have to choose the option 1 and give the details of the main applicant but the main applicant applying for ILR and there is no ILR option to choose the visa catogory for the main applicant . if i choose Tier 1 . then ask for the main applicant visa expiry date and total to pay show 0 .

i Tried the other way as below :
i filled the details of the pbs dependant and then have three option to choose. instead of sellecting option 1 i clicked option 3 . these details are correct . now the total to pay is 1200 .

my doubt is if i do this way and pay the IHS will that be fine . i know this is just a forum and memebers can give ony their openion , still if any one can put there valuable comeents in this that would be a graet help .
