Why UTR letter?
U need to provide Corporation Tax doc to show ur business is registered with HMRC. Any of these CT41G/CT600/CT603 etc.
For business u need
1. Business bank statement/bank letter
2. CT
3. CAR
For Job
1. RTI FPSs
2. Wage slips
3. Employees ID

Nothing more, nothing less.

Its best to send CT600 and CT41G. No need to send any other. You already have been advised by other members above.

There is nothing to describe.

You are not claiming hours. You are only claiming whether it meets a full time job and once the total is more than 30 hrs, it is full time.

In the set o form
Job details
Job title
Then we go to employee details
Employee name

Then it ask do you want to add another employee. yes or no
My question i have two part time employee working under same job title " Assistant". I will add my second employee in the same job title?

One more question, I had one employee who worked with us from Jan 2015 till April 2015 then left. He was full time I have to mention in the form or not?

You should have the passport copy showing the biometric details of the employee plus photo and signature

yes because you are combining to part timers to make up a fulltime so do the same as you said above

By permanent residence you meant he has ILR? As your employee should have a settled status in UK that is ILR

I am not sure if this is accepted. The employee should have ILR to be considered as settled.

Family members of EU nationals hold Permanent residence cards not ILR.
You ll also need to send a copy of the spouse's passport and marriage cert
Read the guidance..

You only need your employees passport and PR documents to prove they are settled.

If an employee holds PR, they are settled, i.e. equal to ILR and no longer dependent on their EU sponsor.
