With regards to CT41G. Do we need to get new copy with current address on it?
My CT41G is very old one from company registration time and it has old address on it. I live at different address now.

Does not matter if CT41G is at the old company address. That is what you need.

Just out of curiosity, looks like you are doing business from home? Were you interviewed at extension time? How long your extension took?

Thanks for clarification tier11417.

I do have office where employees work from. But registered address is my home address for the business.

My extension application was refused due to FPS document missing start date for employee's but then fresh application was approved 3 months later.

In total it took just over 6 months to get visa sorted.

Btw the law has changed now FPS for start date is not required.

Is there just one consent form to sign by main applicant?
I could only find 3 forms to sign in total as below.
1. Part 1: Consent for the Home Office to verify application information - To be signed by main applicant only?
2. Part 2: Consent for the Home office to verify third party information - To be signed by partner if supplying joint utility bill or joint bank account statement?
3. PART 3: Consent for the Home Office to verify information from third party sponsor - to be signed by third party if sponsored by them

is that all or I am missing any other consent forms to be signed?

My extension was refused due to missing start date from FPS reports. This time my accountant is not providing FPS report which shows everything on it. he is saying the software is changed so its not possible to produce same report from new software.

What about other details like end date or no of hours per week etc. Do those details need to be in FPS report?

There is no no. of hours on FPS. However some FPS software show start and end dates but its not necessary

Tier 1 guidance pg 31:


“(a) Printouts of Real Time-Full Payment Submissions showing you are complying with Pay As You Earn (PAYE) reporting requirements to HM Revenue & Customs, in respect of each relevant settled worker as legally required, and have done so for the full period of employment used to claim points. These must show every payment made to each settled worker as well as any deductions”
