on question 1.2 Family name it states:-
The name by which your family is known (also known as surname/last name) and must be as recorded in your passport. Please do not use titles such as Snr.

Now my wife has an identity card with her new married name her surname is now my first name due to customs in that country rather than last name. Now she will be applying for her passport and its absolutly essential she has her identity card which the name on it will be her new married name. So what am I supposed to put on the VA4 form? her previous surname or her new married surname which is on her passport and identity card?

I think you have already given the answer to your question ..... the surname :-

must be as recorded in your passport

And as you have told us :-

her new married surname which is on her passport and identity card

-: clearly that is the surname to go on to the form VAF4.
