List of documents needed to apply ILR post maternity leave

Hi,

I am about to finish my 5 years tenure on Tier II visa in August 11th 2019. SO i wish to apply for ILR on 15 July 2019.
Can i know the list of documents that i need to submit for the application.
I was on maternity leave for the past 9 months and i rejoined work in mid of April. So I wish to submit payslips of April(half), May, june. will those be sufficient or do i need to submit payslips before i started maternity leave?
Also i was not paid full in last three months before i started my maternity leave as i took unpaid leaves for not able to go to work daily during end of my pregnancy. Should i inform about my unpaid leaves anywhere in my application?
Do i need to give details about unpaid leaves that i have taken in these 5 years?

Do we need to inform about unpaid leaves that are taken while on Tier II general visa during ILR application?
