If you look at the UK visa guide page, you will see that she qualifies for transit without visa (TWOV) if she is transiting through UK to a third country. However, she doesn’t qualify for TWOV on her way back from a third country through UK to a destination inside common travel area (Ireland, Isle of Man etc). Whether she’s your spouse or not, it doesn’t make any difference as she holds a normal Stamp 4.

In order to transit through UK to CTA, she needs to have a BIVS endorsed visa issued by Ireland. The now-abolished Irish re-entry visa does not come with this endorsement.

Having said that, I myself have travelled through UK holding a Stamp 2 a few years ago, and I know quite a few non-EU Irish residents have done this.

I just wouldn’t recommend this approach if you can transit via another country (e.g. Amsterdam, Paris, Copenhagen, Helsinki, Middle East etc), because when you transit through UK to Ireland for example, you have to go through border check in the UK airport, and it could take more than an hour in main airports like Heathrow. Plus it would completely depend on the immigration officer’s mood as she doesn’t qualify to TWOV.
