EEA family member - Family Permit to ILR

I am new this post and would like to know the ffg please.
I have called the homeoffice with regard to this and was told to fill in Form
EEA4.
My husband is a portugues national and I am south african.
I have been in the UK since 2003 and have the 5 yr family permit stamped in my passport.

I would now like to apply for ILR for our daughter and myself.
In the form there is requests for various personal documents which I do not have a problem with.

My husband has been self-employed since our arrival in the U.K.
Now my questions are as follows:

1. Which documents will he need to submit apart from account records and bank statements passports and residency docs.

2. We've moved house twice - Do we need to provide utility bill from the old residences or old tenancy agreements as proof that we've been exercising treaty rights for the past 5 years?

3. Would I need to provide proof that I've been working permanently for a year now. Letters of appointment etc.

4. Would my daughter need a letter from her school?

5. Which other documentation would anyone suggest us sending that would aid the application without unnecessary delays and requests from Homeoffice for more documents.

I would highly appreciate some advise concerning this matter as I would have to apply within the next week or so.
