Hello,

I came to UK in Sept-2013 on T-2 General visa and Got my ILR on 01-08-2018.

My Wife joined me on same date but had a break 2014 and had joined me back in August-2015.
Her current visa category is "PBS Dependent". It expires on 09-09-2019.

I am not sure which visa category, I will have to use to extend her visa.

Shall I apply under "PBS Dependent", It asks about COS, maintenance from sponsors etc. which I don't find relevant as I have got the ILR and does not have a sponsor.

Can someone point me to the form link please.

Yes she applies for a 3 year extension as a PBS dependent.

You simply put the details of your last tier 2 job in the form. This is likely only to link her app to the tier 2 rules.

If she changes visas to FLR m, her clock for ilr will reset to zero.

90days maintenance funds at £630 is mandatory in either of your bank accounts.
