My Question she need to do English course for first Extension?

If yes then which course requires. ?

Yes, your wife requires A2 level English from a test/test provider on the Home Office approved list. Guidance is in the link below:

What English test she used in obtaining her initial spouse visa? If it was at least of A2/B1 level and is on HO list then she might not need to redo it.

You only have the option of Super Priority Service (costs an extra £800) or standard service.
