Applying for a Replacement BRP

Hello,

I have applied for a replacement BRP in the UK as it was stolen when i was in france last month. Though when filling the application online i have mentioned it as stolen with the details like when it was lost and where the police complaint was registered but in the checklist only address proof and consent form has been asked to submit.

1. Should i submit the police report? If yes, should it be translated to English?
2. Is it enough to provide one address proof?
