British citizen marrying pregnant Polish woman

Good afternoon.
I'm a British citizen and have been my whole life. My Polish partner, who has been living and working in the UK for at least the past 5 years, is currently pregnant. We are getting married to each other in November and we would like to know what the process is for to change her to take my surname at time of marriage and what she needs to do to get her Polish passport and ID updated. Been searching online for an answer but getting confused with all the red-tape, especially now we are close to Brexit and getting married a few days after we pull out Any advice will be appreciated.
