Life in UK test Validity

Hi guys, I am looking some information about my life in the UK test report validity for my passport application. Well I have received my ILR in 2011 and had supplied my life in the UK test report during my application in 2011. Currently I wish to apply British passport and I need to provide life in the UK test report as part of the application. Can anyone tell whether life in the UK test report (which was taken in the year 2011 and I have still with me) is still valid for the current passport application.
Second thing is, about professional reference - what would be the easiest method to get the reference. Thanks.

You need to find someone, there must be a person such as a director of a limited company, manager of a VAT registered company, minister of religion etc etc.

Your professional referee does not have to be British, they can be of any nationality.
