PBS dependant (partner General Tier 2) switch to Tier 2 General

I would be really greatful if someone knowledgeable can answer me:
1. I am on Tier 2 partner pbs dependent visa since 1 year, i am working as a non training doctor in NHS. Now, i have got training job, where do i apply from? Here or back home?? Job is on shortage occupation list.

2. Second thing, can i apply in nov as i am traveling back home as job start in feb 2020 and come back in early december, and keep doing my previous job. And start my new job in feb. My pbs dependent expires in august 2020. Or should i go back in January to get it done, as i have read that I can’t come 14 days earlier than start date of job. Written in CoS
