Extending Tier 2 for spouse

My situations are as follows:

1. I had Tier 2 visa until April 2019 (I got ILR in April), so my wife has got Tier 2 Dependent visa - which is valid till 31st March 2020.
2. I need apply for my wife's Tier 2 Dependent visa extension. Which visa application form I should complete? If anyone can give me the link, that would be great.

2. She applies for a PBS dependent visa extension. It will be a 3 year extension. You need £630 maintenance funds evidence for 90 days.

I had Tier 2 (General) visa until April 2018, so my wife also came to the UK as a Tier 2 (Dependent).
I have got ILR in April 2018. I did not change my wife's visa as it is valid till March 2020.

Questions:
How can i extend my wife's visa now? Would that be under Tier 2 (dependent) extension?

You need to meet the 90 days maintenance requirements in either of your bank accounts.
