HSMP visa extension

Hi All,
My current visa expires in first week of Oct 2008. As per Jr judgment do I still need to fill Tier 1 general application form along with all proofs for the point claimed (Except English language requirement) or there will be some other application form for the Visa extension?
Do I need to send my all documents as proof like degree, earsning and employment related (all those document which was sent during initial hsmp application in Aug 06) once again along with tier 1 application.
What should be the perfect time to apply. I had stayed 5 months on work permit before I got HSMP.
