Switch from ICT dependent to Tier 2( within UK)

My visa ICT spouse visa expiring next year.
My consultant told me that switch to Tier 2 can be only processed outside UK from home country.

Do you think home office can give any exception to people who are pregnant and would anxious to travel to home country for visa?

If you intend to stay in the UK
If you decide to stay in the UK, you should apply for the necessary leave to remain in the UK. You’ll also be able to submit an application form from within the UK where you would usually need to apply for a visa from your home country.
