British citizenship : Help with online application

Hi All, i am going to apply for British citizenship next week, really just wanted to confirm few things before i send my application.

Apologies as these questions were asked before ,but my confusion is because of my immigration history to the UK.

Here are my questions

1)Under the section immigration that have you made any previous UK immigration application in the UK or abroad.

Student visa : I came to the UK in 2005 student visa , student visa extension and Fresh talent work visa

Tourist visa : 2009 and 2012

Tier 2 : 3 extensions

ILR

Do I need to write details of all these applications including tourist visa to the UK ??

2) When you first entered into the UK

In my case 2005 ??

3) References : Up to the stage 4 payment I couldn't find the reference forms , after the payment this forms would be available ??

4) Do referee need to write name and DOB of an applicant and then attach the picture on the application, didn't understand the rational behind it
