Referee Declaration Photo Question

I've read the instructions on the referee declaration PDF document and it's unclear to me whether the photo needs to be of the applicant or the referee?

Could anyone please confirm my understanding?

It needs to be of you (the applicant) - NOT the referee....

Also I've read that:

There's no need to write anything on the back of the
photo before gluing it to the form, this was necessary when the documents were sent as hard copies by post, in case the photo became unstuck, but it's not necessary if the completed form is scanned and uploaded in digital format.

Can we confirm that this is true?

My application was in paper form so can't comment on whether this is a new instruction.
In case you send the form to your referees it might still be an idea to do that.
But for scanning I should think it's not necessary as you keep the original.

You should still write the details in the back of the form. UKVI can and often do ask for originals to be sent to them.
