IGS Dependant visa help

I want to apply IGS Dependant Visa for my wife over India.Before My wife student visa was refused twice because I showed My uncle as a sponsor and the paper work was not good. I read out all the requirements needed from website now. I got My IGS visa on 28th July this year valid until 22nd June 2009. I am working full-time even before applying to IGS. And also, I was getting my salary in a different bank account. Now, I am going to switch It to my bank from this month which I showed to apply IGS.

Also, I am thinking to apply next month with two payslips with a balance around £3,000. I got a employment letter saying I am working Full-Time with the company. With that I am going to show the following documents as follows:

Tennancy Agreement (rent showng £325 per month)
6 months Bank statement ( balance would be around £3000 end of september)
2 wage slips (or) even 3
a letter from local Council stating I got sufficient space in my flat
passport copy (recent page with IGS)
Sponsor's letter( My covering letter)
Marriage Certificate
My wife's bank statement ( around £1200 = Indian rupees)

I am bit confused which application form I need to Include
Could you please help with this.
