How to apply for EC from Mumbai

Hello Everybody,
I got my HSMP approval on 18/04/08;now I want to apply for EC in Mumbai.

Can anybody pls tell me which form I'll have to fillup and which documents are necessary for applying for EC-in fact it would be great if someone can tell me the whole process of applying for EC.
