Question about IHS

Hi all,

I was just wondering of someone could clarify a question I have surrounding the IHS.

My fiance and I are currently deciding on whether to go down the fiance visa route or the spouse visa route. The deciding factor is going to be cost, I was wondering if anyone could tell me if you have to pay the IHS for both the initial fiance visa, then again for FLR, i.e spouse visa?

Its quite confusing on the gov page and if anyone who has gone through this route could help a gal out I'd be very grateful.

Who needs to pay
You usually need to pay the healthcare surcharge if you’re applying for a visa or immigration application:

for more than 6 months, if you’re applying outside the UK
for any length of time, if you’re applying inside the UK
You do not need to pay if you’re applying for a visitor visa or to remain in the UK permanently.

If you’re from the EU, Switzerland, Norway, Iceland or Liechtenstein, you only need to pay if you’re applying to come to the UK from 1 January 2021.

You still need to pay even if you have private medical insurance.

Those on a fiance visa have to pay to use the NHS, so cost out how much insurance will be if she has a fiance visa, until she receives a FLR(M) visa. 50% is added to the NHS bill if there is no insurance. NHS treatment can be refused if the person does not pay first: they cannot refuse treatment to save a life but the person is still billed and 50% is added. NHS debt of £500 plus, can lead to a refusal of a vise to remain or enter the UK.

You will need to pay for any NHS care you get at the point you use it - unless it’s a service that’s free.


That link also gives a link to what services are free for visitors. All the expensive stuff is billed, stay on a ward, operations. etc

Also add in to your costs-
that those on a fiance visa are not allowed to work.
that time on a fiance visa does not count towards her 5 years for ILR.
