question regarding tier 1 application forms (its urgent)

Hi,
We are living in usa and my husband applied for tier 1 application and for having our biometrics date he had to fill out the online forms and when he filled online forms there were" vaf 9" form that he filled but after that he got the biometrics date and application was completed and they charged the fees but that online application didn't asked to fill 'pbs apendix 1" form.my question is that how should we add that pbs "apendix 1" form to our application.The date that they gave to my husband is 4th of october so could any one please tell me the answer of my problem.

my 2nd question is that is there any way that we can expedite our application so that our case would done fast?
