Please provide suggestion and guideline in filling HSMP EXTN FORM

q.25 Name of employer ?
q26. saws appl?
q27 your job title ?
q.31 what is your immigration status in UK
q33. if your partner /children currently living ? if not present in uk ?

do we have to send payment detail form and page 7 to 14 of form flr ied ?
