I have had my visa application refused due to an erroneous or expired COS. However, my wife also was denied as dependant based on the aforementioned reason and also submitted a photocopy of our wedding certificate.

Sir, I intend to reapply after getting a new COS, should I attach a sworn affidavit of my marriage certificate and the original copy of it?

Also, should will I mentioned in the question section why she was refused?

Please I need clarity on this as I submitted a photocopy of my lost wedding certificate and the later requested additional document I then sent the new truly certified copy of my wedding certificate.

This is the exact wording of the refusal letter as follows:

Reasons for Decision
• Your application has been refused because you do not meet SW 30.1 (a) and (b)
• I am therefore not satisfied that you are the child of a person who has permission as a skilled worker or is at the same time applying for ( and is granted) entry clearance or permission on the skilled worker.
For my wife's decision
• I am there not satisfied that you are the partner of a person who has permission as a skilled worker

Please I want to reapply after getting another cos from the same employer after sorting out the cos issue.

Please advise me.

You need to post what the whole refusal letter states so that it can be read in context. What you posted is only a portion of the letter. Post all of it taking out personal information and names.

Your wedding certificate is not the issue unless it was specifically stated as a reason. The dependent applications would automatically be refused because your application was refused.
