Does the processing time start from application & payment or fingerprinting

My missus applied for a visa extension recently. Since I am long term British, the house and financials are all in order, it ought to be straightforwad.

But she could not make the trip for the fingerprinting an photo until about 4 weeks after the initial payment.

At what point might the processing time start, and say the application was 5 September but the Fingerprinting was not until 2 October, when might a decision be made and a new BRP issued?

I would really like to arrange some travel out of the country.

The processing time starts after biometrics.

Keep in mind you cannot leave the country when application is pending (otherwise it'll be withdrawn).
I would very much advise you to postpone your travel plans until your wife has new BRP in hand - it may take a few months for a decision.
