It's better to inform them now before getting the CoS than after.Normally applicants inform the new employer about any notice period, so they can issue the CoS - including tentative start date, etc - taking into account that period.
Apply to update your visa
You must apply online for a Skilled Worker visa. You’ll be asked if you’re applying for a Health and Care Worker visa as part of your application - make sure you choose ‘yes’.
